SummaryJob Summary: Identify specific learning needs of the community. Collaborate with key customers to strategize learning plan and program content. Design and develop appropriate learning activities to meet stated objectives. Deliver educational programs utilizing age-appropriate learning principles. Perform ongoing assessment of programs to modify content based on evaluation results and customer feedback. Assist in screenings, programs and community outreach activities as requested. Accurately assess medical history, educate and prepare patients for screenings/treatments. Document and track patient encounters using appropriate tracking tools. Develop and maintain a strong working knowledge of available community and hospital resources and current trends in healthcare, to ensure information provided is accurate, relevant, and up to date. Use of high-risk screening criteria and nursing assessment skills to identify high risk patients and determine patient and/or family strengths and need for Health Navigation services relative to effectively managing the disease process. Maintain strong working relationships with medical providers, representatives from community agencies and institutions to ensure access to services for patients by conducting an assessment, identify gaps in services and support quality improvement and program growth. Assisting patients in navigating through health care and community support systems to achieve quality care and follow up to improve or maintain quality of life. Contact with the patient is mostly on the telephone but may also include in-person visits in the hospital, emergency department, home visits, and/or doctor office visits. Develop care plans/care alerts on an individual basis to address health care needs for individual patients in coordination with their medical providers and other health care team members, while informing hospital staff of important history and barriers to the patient's care. Enhance personal and professional growth and development through participation in educational programs, research of current literature, in-services, meetings and workshops.
