Optometrist
An Optometrist is needed in Portage, IN.
Known for its scenic access to Lake Michigan and the Indiana Dunes, Portage offers a family-friendly community with plentiful outdoor recreation, good schools, and proximity to Chicago. This opportunity is with a mission-driven FQHC that delivers integrated care to underserved populations across Northwest Indiana.
Salary and Benefits:
- Salary Range: $120,000 $145,000 (based on experience)
- Schedule: hours per week
- Vacation: 120 hours per year
- Personal Time: 40 hours per year
- CME: 40 hours per year and $2,500 allowance
- Holidays: 10 paid holidays annually

Responsibilities:
- Deliver comprehensive optometric services in an outpatient clinical setting
- Diagnose and manage ocular conditions such as glaucoma, cataracts, and macular degeneration
- Prescribe eyeglasses and contact lenses
- Perform contact lens fittings and follow-up care
- Collaborate with ophthalmologists and other specialists as needed
- Ensure departmental quality assurance and protocol compliance
- Engage in administrative meetings and contribute to team-based care initiatives
- Maintain licensure and documentation in accordance with regulations
- EMR System: Dentrix
- Reports To: Director of Optometry
Requirements:
- Doctor of Optometry (OD) degree from an accredited institution
- Active Indiana Optometry License
- Indiana Optometric Legend Drug Certificate
- Current CPR certification
- Minimum of 2 years of clinical experience preferred
- Strong interpersonal, communication, and organizational skills
- Passion for community health and integrated care models
