Job Title: Home Health LVN/LPN - DaysLocation: Brockton, MA and nearby areasJob Description:We are seeking a dedicated Home Health LVN/LPN to provide compassionate care to patients in their homes. Proficiency in HCHB/OASIS documentation is essential.Key Responsibilities:• Deliver quality nursing care• Complete same-day documentation• Manage 20-25 cases weekly• Attend weekly in-person meetings• Be flexible to float as neededRequirements:• Active MA LVN/LPN License• BLS Certification• 2+ years of home health experience• Proficient in HCHB/OASIS, including Oasis E• Manage 26-28 productivity points weekly• Reside within 30 minutes of territoryContract Details:• 13-week contract, 40 hours/week• No on-call or overtime• Mileage reimbursementAdditional Information:• 2-3 days orientation• Flexible hours, 8 am to 5 pm• Any scrub color
