Location: Fort Defiance, AZ 86504

Contract Type: Travel

Shift: M-F, 8-5/730-430, no holidays no weekends.

Contract Length: 13 weeks

• Care Coordinator (case manager) nurse positions within our Primary Care clinic.
• Shift: Weekdays Monday through Fridays from 8a to 5p, with One (1) hour lunch, no weekends or holidays.
• Requirement: Three (3) year Case Management Experience.
• Certs: BLS & AZ/Compact RN License
