Monitor and support the health and wellness of individuals served
Administer medications and maintain accurate documentation
Deliver physician-ordered treatments and personal care
Conduct assessments and contribute to Individual Service Plans (ISPs)
Train staff on infection control, safety, and medication administration
Coordinate with physicians and community health professionals
Educate individuals on health topics like self-care and smoking cessation
Associate's Degree in Nursing or a state-approved LPN certificate
Current LPN licensein Massachusetts
1 year of nursing experience with similar populations preferred
Excellent communication skills and a compassionate approach
