Implementing a comprehensive social services program that provides support services for patients and families in the center. Demonstrating teamwork characteristics by building a positive team approach. Ensuring the quality of social services rendered by the center. Understanding and abiding by established policies of the health care center and interpreting them to concerned parties. Providing direction to department in meeting center's goals and objectives. The qualified applicant for this position should possess: Degree in Social Work or related human services major. Knowledge of the medical case management process, needs of geriatric patients in health care settings, working knowledge of funding resources such as Medicare, Medicaid, Managed Care and other private insurances, MDS and EHR.
