The Community Health Worker (CHW) will be responsible for assisting patients and their support team, to navigate and access community services, other resources, and adopt healthy behaviors. The CHW supports the healthcare system providers and staff through an integrated approach to care management and community outreach. The CHW will provide social support, informal counseling, and advocate for the health needs of individuals and their community. Community outreach and collaboration with community agencies are essential to this role. The CHW will participate in the process of collecting and maintaining health-related social needs screening data. Qualifications. Minimum Qualifications. A minimum of one year experience in a healthcare setting working with patients, healthcare services, medical groups and payors required. A minimum of 1 year experience in community outreach, advocacy work within the healthcare/community. High School Diploma.
