The Health, Outreach, Prevention, and Education (HOPE) Case Manager plays a vital role in empowering individuals to achieve optimal health and well-being through community-based support services. This position is responsible for conducting intake assessments, developing personalized care plans, making referrals, re-evaluating progress, and coordinating discharge planning for individuals enrolled in HOPE Services. With a strengths-based, client-centered approach, the HOPE Case Manager ensures services are delivered with compassion and effectiveness, helping clients navigate barriers to care and access critical resources. Additionally, this role is responsible for program implementation, evaluation, and compliance with funder requirements while fostering strong partnerships with key stakeholders to enhance community impact.
