Family Health Centers at NYU Langone, a designated Level 3 Medical Home, is the largest federally qualified health center network in New York State, and the second largest in the nation. It includes eight primary care sites, 40 school-based health centers and dental clinics, four day care centers, the nation's largest dental residency program, and New York State's largest behavioral health program. The network also includes 12 community medicine sites providing care to disenfranchised New Yorkers, comprehensive HIV services, chemical dependency programs, and a family support center that offers educational, vocational, and other social support programs.

Position Summary: We have an exciting opportunity to join our team as a Program Counselor-Social Work. In this role, the successful candidate The Program Counselor supports the Seniors In Touch program in the provision of person-centered social adult day services, and responds to the psychosocial needs of program participants and caregivers. Seniors In Touch is part of the Family Health Centers at NYU Langone Community Based Programs and located in Bay Ridge, Brooklyn. The program provides social, recreational, and therapeutic activities to older adults living with cognitive and/or physical impairments, as well as caregiver respite. The goal of this position is to enhance existing participant and caregiver support services offered onsite. The Program Counselor will provide services to participants and caregivers including, but not limited to, screening, intake and bi-annual biopsychosocial assessments, care planning, crisis intervention, supportive counseling, case assistance, information and referral, group work, and discharge planning.

Job Responsibilities:

  • Provides biopsychosocial assessment and care planning at intake and required six-month intervals, and completes necessary documentation in a timely manner. Assessments may take place in program office or participant home environment.
  • Maintains professional documentation as defined by organizational policy and procedure and funder requirements.
  • Assists in the collection and reporting of timely, accurate, statistical data.
  • Provides psychosocial counseling as identified through assessment and care planning for patients and families/support systems to enhance participation and respond to presenting needs including, but not limited to, dementia-related behaviors, illness and disability, and home care needs.
  • Performs other duties as assigned or volunteered in alignment with mission, goals, and values.
  • Utilizes knowledge of community resources to refer patients to appropriate services to ensure continuity and quality of care.
  • Completes referrals for transitional and post-discharge services to meet patient needs and safely transition to the next level of care.
  • Plans and implements caregiver support groups and education as well as group engagement with program participants around meaningful activities and identified psychosocial needs.
  • Provides case assistance and information/referral regarding concrete services, benefits, and entitlements, as well as advocacy and assistance in navigating complex systems of care.
  • Provides supportive counseling, crisis intervention, and referrals to protective services as needed.
  • Collaborates with the Community Based Programs and Services for Older Adults team, and other relevant stakeholders, to assess and plan for appropriate care and services across the continuum.

Minimum Qualifications: To qualify you must have a Masters Degree in Social Work (MSW)Current NYS Social Worker License and Registration (LMSW or LCSW). Required Licenses: Lic Master Social Worker

Preferred Qualifications: At least 1 year post-masters experience, preferably in the field of dementia care or other older adult services.

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $57,752.53 - $78,652.00 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
