OF ROLE:As part of the ACT Teams the LPN will take vital signs, such as blood pressure, temperature and weight, follow up with community health care providers to ensure medical treatment plans are documented, monitored and followed, and provide CPR/ First Aid. Part time schedule (3 days a week; flexible on days)ESSENTIAL DUTIES & RESPONSIBILITIES:The LPN assesses, monitors, and ensures access to coordinated medical and mental health services. This position requires you to conduct home visits on a regular basis with 50% fieldwork/travel. You will work with consumers in filling prescriptions, coordinating appointments refer residents to emergency care if needed, work with emergency responders/emergency room staff to coordinate emergency treatment, coordinate treatment, and discharge planning with hospital staff.
