Job Description:We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our Gastroenterology team in Louisville, KY. The LPN will play a vital role in delivering high-quality patient care, collaborating with interdisciplinary teams, and supporting patients through their recovery and functional improvement.
Responsibilities:
- Perform daily comprehensive assessments and prioritize care based on patient needs.
- Administer medications and treatments as prescribed, documenting all interventions and progress.
- Collaborate with physicians and interdisciplinary teams to revise care plans and communicate changes in patient status.
- Educate patients and families on self-care, medications, and follow-up care.
- Maintain compliance with infection control, safety standards, and HIPAA regulations.
- Participate in performance improvement initiatives and complete special projects as assigned.
- Ensure proper functioning and cleanliness of patient care areas and equipment.
- Attend shift meetings and complete mandatory training and competencies.
Qualifications:
- Completion of an accredited LPN program (required)
- Current LPN license in the Commonwealth of Kentucky (required)
- CPR/BLS certification through the American Heart Association (required)
- Prior acute care or rehabilitation experience (preferred)
Skills & Abilities:
- Strong knowledge of nursing theory, medication administration, and lab value interpretation
- Effective communication and active listening skills
- Ability to collaborate and delegate appropriately within a healthcare team
- Time management and organizational skills
- Proficiency in age-appropriate care and medication dosing, including pediatric and geriatric populations
