Scope of Position: Residential Scattered-Site Case Managers support formerly homeless adults living with serious mental illness and/or substance use disorders. You will provide comprehensive case management services, assisting clients in accessing housing, healthcare, benefits, and community resources, while fostering a supportive, trauma-informed environment. Your work will empower individuals to develop skills necessary for maintaining housing and achieving stability. Building trusting relationships and offering personalized support, you will guide clients toward independence, improving their quality of life and fostering hope.
Essential Position Functions:
- Provide case management and interventions for high-risk clients, including assessments and referrals, primarily in clients' apartments and community settings.
- Identify clients' strengths, needs, abilities, and goals, and create Support Plans in the electronic client records system.
- Use Motivational Interviewing techniques to help clients develop skills needed to maintain housing. Support natural supports and include them in Support Plans and meetings.
- Complete required documentation for every client contact, following agency policies and regulatory guidelines.
- Co-facilitate groups in areas such as wellness, housing stability, employment, relationships, and healthy decision-making.
- Assist with housing applications, referrals to medical and mental health care, benefits management, escorting clients, ensuring compliance, and coordinating care with external providers.
- Attend case conferences with the High-Risk Coordinator to develop interventions.
- Participate in staff meetings, supervision, and trainings.
- Assist in preparing for audits, ensuring compliance, managing admissions and discharges, maintaining records, and incident reporting.
- Collaborate with other departments, external agencies, and treatment providers.
- Work with consumers, families, community tenants, and staff respectfully, with cultural sensitivity.
- Conduct apartment inspections, provide escorts via public transportation, and ensure safety.
- Report serious incidents and complete incident reports per policy.
- Account for consumers, conduct searches, and file missing person reports following policy.
- Provide crisis intervention using de-escalation techniques.
- Perform additional duties, potentially supporting other programs.
Requirements:
- Bachelor's degree required.
- Experience with mental health issues, crisis intervention, and managing trauma-triggered behavior.
- Excellent time management and organizational skills.
- Ability to work effectively in a team, remain stress-resistant, flexible, creative, and receptive to feedback.
- Willingness to perform CPR, First Aid, AED, and Naloxone administration.
- Knowledge of the AWARDS system.
- Bilingual in Spanish is a plus.
- Experience working with veterans or direct veteran experience is a plus.
