Case PlannerJuvenile Justice Services Program Location: Nassau County Schedule: Full-Time (35 hours/week), Non-Traditional Schedule About the Program: The Juvenile Justice Services Program at Hope For Youth provides case management, restorative justice conferencing, and clinical services for up to 40 youth referred by the Nassau County Probation Department. These youth may include:
- Persons In Need of Supervision (PINS)
- Juvenile Delinquents (JD)
- Juvenile Offenders (JO)
- JD-Raise the Age
- Adolescent Offenders
Our program focuses on family-centered treatment services that have proven effective in reducing delinquency risk and avoiding out-of-home placement. Position Overview: The Case Planner plays a critical role in supporting youth and families by improving individual functioning and family dynamics. This is a field-based position requiring weekly contact with youth and their families throughout Nassau County. The Case Planner also delivers direct services such as:
- Case management
- Life coaching
- Parent skills training
- Coordination with schools, courts, and other community partners
The Case Planner reports directly to the Program Supervisor. Key Responsibilities:
- Maintain weekly contact with youth and families (including evenings/weekends), utilizing your own vehicle
- Implement all components of the program model for a caseload of up to 20 youth
- Provide case management, life coaching, and parent skill training
- Make collateral contacts with schools, service providers, and justice system representatives
- Conduct CANS 2.0 assessments (initial and follow-up)
- Attend school meetings and court appearances as needed
- Regularly review and update service plans
- Participate in transition and discharge planning
- Complete timely and accurate documentation
- Attend supervision, treatment team meetings, and ongoing trainings
To Be Successful, You Must Be Able To:
- Adhere to agency policies, program procedures, and ethical codes of conduct
- Support and empower families to maintain youth in the home
- Achieve positive outcomes as outlined in NYS, County, and agency standards
- Be flexible and responsive to the needs of youth, families, and the program
- Complete all tasks and responsibilities as required by Hope For Youth and regulatory agencies
Minimum Qualifications:
- BSW or Bachelor's Degree in a related field plus 2 years of casework experience
- OR MSW or Master's in a related field
- Excellent verbal, written, and computer skills
- Ability to work a flexible, non-traditional schedule (some evenings and weekends)
- Access to a reliable vehicle for regular home visits throughout Nassau County
- Bilingual (Spanish/English) preferred
Physical Requirements:
- Ability to read/write reports and log entries
- Walking/standing for prolonged periods
- Bending/stooping, kneeling/squatting, climbing stairs
- Reaching overhead, repetitive wrist/finger movements
- Regular driving using personal vehicle
