A Case Management (CM) Assistant functions under the direction of CM services clinical personnel, assisting with activities not requiring clinical expertise. CM Assistants possess excellent communication, organizational and computer skills. Responsibilities Supports Case Managers and Social Workers as assigned to secure post-acute providers of care for patient post-discharge needs by collecting, entering, and sending referrals, then monitoring for follow-up responses until referral process is complete. Supports CM to provide the Centers for Medicare & Medicaid Services communications and forms to patients in a safe, timely, and effective manner, and facilitates appropriate follow up to enhance the quality of the services to patients. Facilitates multiple care aspects under the direction of the care coordinator for discharge planning such as: assist with referrals and/or appointments to Physicians and/or clinics or coordinates transportation from hospital upon discharge as needed. .
