Job Description :
Essential Duties and Responsibilities include the following:
  • Medical Assistant role is to support both pharmacists and members in managing their health and navigating the healthcare symptoms.
  • Complete initial assessments to enroll members into diabetes disease management program.
  • Conducting close follow up calls to ensure member adheres to goals
  • Complete routine medication reconciliations
  • Request blood glucose levels routinely for pharmacist to monitor and adjust therapies
  • Contact and schedule members in Chronic Disease Management Clinic.
  • Works with members to assist them in navigating the complex healthcare system such as:
  • Assist and educate members with facilitating appoints with providers. If there are no valid authorizations on file then will contact Primary Care Providers office to request them to input an authorization.
  • Ensure member has at least an annual visit with Primary Care Provider(PCP)
  • Ensure member has an annual visit with Ophthalmologist for an eye exam
  • Ensures member has a follow up visit with Specialists every 3-6 months per Specialists request.
  • Ensures member has labs completed every 3-6 months depending on labs
  • Assist members and schedule appointments with quest or providers.
  • Confirms member has medical supplies needed for disease management such as glucometer and supplies for testing blood sugar levels, blood pressure monitors, and scales, etc. If not, team works with patient and assists them in getting the needed supplies.
  • Contact health plan for benefit checks on medications and covered services.
  • Refer members to appropriate services such as Social Work, Health Education and Behavioral Health as needed
  • Assist with connecting to appropriate providers for question or concern.
  • Assists other staff and supports the team approach.
  • Participates in the efficient, effective, and responsible use of resources such as supplies and equipment.
  • Communicates appropriately and clearly to management, co-workers, and physicians.
  • Perform other duties as assigned
  • Prepare patients for provider exam, take vital signs, record patient history and assist in office procedures
  • Greets patient upon arrival
  • Stock exam rooms with supplies and maintain inventory
  • Maintains medical supplies and cleaning/sterilization of the medical instruments and equipment
  • Take and record height, weight, and blood pressure, pulse rate, respiration, temperature and notify provider of unusual changes
  • Ensure that patients have been properly instructed, using the approved written instructions, regarding proper preparations for tests/procedures that have been ordered by the provider
  • Adhere to the Medical Assistant's scope of practice at all times
  • Performs selected clerical and administrative duties, and assists physicians as part of the care team
  • Be willing to learn new procedures or accept new assignments, to maintain office operations all employees will be cross-trained in both the front and back office
  • Must be able to come into the corporate office once a month for our monthly team meetings.
The pay range for this position at commencement of employment is expected to be between $21 - $23 per hour; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, licensure, skills, and experience. The total compensation package for this position may also include other elements, including a sign-on bonus and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered. Employer will consider for employment qualified applicants with criminal histories in a manner consistent with the requirements of the LA City Fair Chance Initiative for Hiring Ordinance. Education and / or Experience:
    • Knowledge of Healthcare and Managed Care preferred
    • Graduation from an accredited medical assisting program required
    • Understanding of HMO processes
    • Proficient in MS Office programs (i.e., Word, Excel Outlook, Power Point and Visio)
    • BLS Certification required within 30 days of employment.
