Social Services Coordinator β Outreach & Program Support

Position Overview:
We are seeking a dedicated and self-motivated Care Coordinator to support care management efforts and community engagement in a flexible, mission-driven environment. This role plays a vital part in bridging outreach and operations, with 40β50% of responsibilities focused on field-based outreach. You will work directly with Managed Care Plans, helping assess member eligibility and supporting warm hand-offs to Care Managers. This position is based in Downtown LA, in a public transportation-friendly office located directly across from Union Station.
Key Responsibilities:
β Conduct outreach to members provided by Managed Care Plans.
β Complete eligibility assessments and coordinate warm hand-offs to Care Managers.
β Collaborate closely with LCSW's to execute care plan components and administrative needs.
β Attend internal and external meetings as a program representative.
β Maintain consistent follow-up and ensure efficient communication across teams.
β Participate in ongoing operations and contribute to process improvements.

Must-Have Qualifications:
β Master's degree in Healthcare, Social Work, or a related field
β MSW strongly preferred β must understand healthcare coordination
β Demonstrated accountability and ability to build trust with vulnerable populations
β Excellent communication, leadership, and team collaboration skills
β Experience with program management and strategic planning
