Open Sky's Behavioral Health Justice Involved program is a new model of care designed to assist Mass. Health Members who are Justice- Involved--- either as current inmates or detainees, or on Justice Services such as probation or parole. The goal of the program is to support individuals in navigating and successfully engaging with health care services and behavioral health services. The program is designed to improve health outcomes and decrease fatal overdoses by providing connections to health care and community services. The Justice Involved Navigator will provide In-Reach and Re-entry support, assessment, education, support and safety planning for individuals enrolled in the Behavioral Health Supports Justice Involved Program.
