Summary: A Community Support Specialist is needed for ICORT Team in McComb, MS. This position will provide case management services to adult participants on the ICORT Team. Some on-call duties required.

ICORT is a program that serves individuals with severe and persistent mental illness in the community. The Intensive Community Outreach and Recovery Team (I-CORT) is a recovery and resiliency-oriented, intensive, community-based rehabilitation and outreach service for adults with a severe and persistent mental illness. I-CORT is a team-oriented approach to mental health rehabilitation intervention and supports necessary to assist individuals in achieving and maintaining rehabilitative, resiliency, and recovery goals. I-CORT services are provided primarily in natural settings and are delivered face-to-face, by phone or video conference with the individual and their family/significant other as appropriate, to the primary well-being and benefit of the recipient. I-CORT assists in the setting and attaining of individually defined recovery/resiliency goals. I-CORT primary treatment objective is to assist in keeping the individuals receiving the service in the community in which they live avoiding placement in state-operated behavioral health programs.

Requirements:

  • Bachelor's degree in behavioral healthcare required
  • Must have reliable transportation to provide community services
  • Must have a valid driver's license with a satisfactory driving record
  • Must be able to obtain DMH case management certification within 60 days of hire
  • Must have auto insurance liability limits of 100,000/300,000/50,000 within 1 month of hire
  • Behavioral healthcare experience preferred
  • Excellent communications skills required

Responsibilities:

  • Assists with medication monitoring and system navigation.
  • Assists with outreach of program and case management.
  • Demonstrates knowledge of diagnosis, signs and symptoms of relapse, medication information, and knowledge of community resources to explore all alternatives to hospitalization.
  • Facilitates Person Centered Planning meetings.
  • Provides advocacy, consultation, and educational services.
  • Provides outpatient social services linkage, emergency intervention, and crisis intervention services.
  • Provides services to clients through home or community visits.
