A Brief Overview
Performs utilization management activities and care coordination across the continuum of care in collaboration with the physician and other members of the health care team.
Essential Functions
Develop and maintain a positive and credible relationship with patients, physicians, and nursing staff to ensure a team based approach and an effective continuum of care for the patient.
Identify chronic frequent and complex cases and proactively assist in developing a care plan to ensure the appropriate level of care is provided to the patients and family.
Ensure patient follow-up and follow-through with patients medications, visits, and care directing to community resources as needed
Maintain up-to-date knowledge of community resources available to patients and families and refer appropriately
Collaborate with physicians and other medical staff to promote and maintain a follow-through culture with patients inside and outside of the clinics
Proactively reach out to known frequent and complex cases to ensure that the quality of care is obtained.
Incorporates and counsels on the correct and consistent application, interpretation, and utilization of health care benefits
Monitors the progression of the plan of care and facilitate discussions with the multi-disciplinary teams.
Provides referrals for financial and supportive needs.
Gathers information from patients/families.
Shares appropriate resource materials and information with patients/families.
Obtains necessary release of information authorization.
Provides follow-up as deemed appropriate.
Documents interventions appropriately in chart.
Conducts psychosocial assessments related to the type of illness/needs present:
Screens patients in assigned areas daily.
Establishes professional working relationships with staff/families.
Collaborates with multidisciplinary team to understand the medical needs of the patients/families.
Appropriately documents assessment in chart.
Formulates patient goals on multidisciplinary/daily goal sheets.
Provides appropriate patient education and documents appropriately.
Provides assessment and coordination of suspected child abuse/neglect cases.
Assesses psychosocial dynamics.
Communicates with medical and nursing personnel regarding assessment and plan.
Facilitates referral and provides ongoing communication with CPS, law enforcement, and county attorney's office.
Documents pertinent information and completes all reports in a timely fashion.
Prepares and presents cases at Children's Advocacy Team. Attends on an as needed basis.
Provides consultation/interventions regarding legal issues such as determining guardianship and assisting with adoption cases.
Consults with Compliance Officer/Legal Counsel as necessary.
Provides appropriate patient education.
Provides supportive counseling to patients/families, based on department need and license scope of practice
Assists families with problem-solving and utilizing their positive coping strategies.
Provides interventions which reflect knowledge of counseling technique and is enhanced by disease specific knowledge.
Documents interventions appropriately in chart.
Facilitates support groups as needed.
Education Qualifications
Bachelor's Degree from an accredited college or university in Social Work or related field Required and
Master's Degree from an accredited college or university in Social Work Preferred
Experience Qualifications
Minimum 6 months experience with care coordination, direct patient care delivery & case management, or social work Required and
Experience in a pediatric clinical setting Preferred
Skills and Abilities
Knowledge of the Nurse Practice Act, The Joint Commission, HIPPA & all other applicable federal/state/local laws and regulations
Strong communication and customer service skills, problem-solving, critical thinking, and clinical judgment abilities
Intermediate knowledge of word processing, computer navigation skills
Ability to interpret, and use analytic data in day-to-day operations
Knowledge of healthcare benefits associated with various business lines (e.g., Medicaid, etc.)
