Assess, plan and implement care strategies that are individualized for children and youth in a family-oriented context (e.g., considering reunification and/or other permanency planning needs, needs for adoptive children, and needs for transition-age youth with alternative permanency plans) and directed toward the most appropriate, lease restrictive level of care Identify, initiate, coordinate and monitor referrals for psychosocial, behavioral and medical services Manage the care plan/plan of service throughout the continuum of care as a single point of contact Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members Advocate for children, youth, and families as needed to ensure the members needs and choices are fully represented and supported by the health care team Provide trauma-informed and disease/disorder specific education to members and caregivers Coordinate with the child ...Care Coordinator, Field, Behavioral Health, Coordinator, County, Healthcare
