Care Manager (RN) – Deltona, FLSchedule: Monday–Friday, 8:00 AM – 5:00 PMContract Assignment - 3 months ++Position OverviewThe Care Manager (CM) plays a critical role in improving patient outcomes by reducing hospital admissions, readmissions, and emergency room visits. Serving as a resource, advocate, and educator, the CM guides patients with complex health conditions through the healthcare system, fostering autonomy and optimal quality of life.   • Prioritize patient care needs during initial visits; proactively address emerging health concerns
  • Meet with patients, caregivers, and families to review and personalize care plans under the Transitions of Care program
  • Track high-risk patients across care settings including acute, custodial, and ambulatory environments
  • Apply evidence-based disease management protocols to improve clinical outcomes
  • Facilitate smooth transitions post-discharge including follow-up appointments, home health, and social support services
  • Maintain clear, consistent communication with care teams, patients, and external facilities
  • Evaluate cost impacts of care decisions using CM metrics and health plan tools
  • Participate in daily huddles and weekly Patient Care Coordination (PCC) meetings
  • Prepare timely, accurate reports for internal tracking and care planning
Required‍⚕️ Ideal Background & Qualifications
  • Active Registered Nurse (RN) license
  • Current Basic Life Support (BLS) certification
  • 3+ years clinical experience in primary care, urgent care, or hospital settings
  • Proven ability to multitask and work independently in a fast-paced environment
  • Strong customer service mindset and communication skills
  • Proficiency in Microsoft Office (including Excel)
  • Familiarity with medical terminology, equipment, HIPAA guidelines
  • Willingness to complete color blindness test
