- Prioritize patient care needs during initial visits; proactively address emerging health concerns
- Meet with patients, caregivers, and families to review and personalize care plans under the Transitions of Care program
- Track high-risk patients across care settings including acute, custodial, and ambulatory environments
- Apply evidence-based disease management protocols to improve clinical outcomes
- Facilitate smooth transitions post-discharge including follow-up appointments, home health, and social support services
- Maintain clear, consistent communication with care teams, patients, and external facilities
- Evaluate cost impacts of care decisions using CM metrics and health plan tools
- Participate in daily huddles and weekly Patient Care Coordination (PCC) meetings
- Prepare timely, accurate reports for internal tracking and care planning
Required⚕️ Ideal Background & Qualifications
- Active Registered Nurse (RN) license
- Current Basic Life Support (BLS) certification
- 3+ years clinical experience in primary care, urgent care, or hospital settings
- Proven ability to multitask and work independently in a fast-paced environment
- Strong customer service mindset and communication skills
- Proficiency in Microsoft Office (including Excel)
- Familiarity with medical terminology, equipment, HIPAA guidelines
- Willingness to complete color blindness test
