POSITION REQUIREMENTS:

1. Current licensure by the Washington State Board of Pharmacy as a Registered Pharmacist

2. Comprehensive clinical knowledge of the practice of hospital pharmacy with experiential training consisting of one or more of the following: Hospital experience (two years preferred), successful completion of an accredited hospital pharmacy residency, and/or Board Certification in a pharmacy specialty

3. Certification in Basic Life Support (BLS)

4. Demonstration of clinical competence

• Demonstration of excellence in interpersonal communication skills, Experience and/or knowledge in drug therapy, and the ability to perform and/or consult in the following areas of hospital pharmacy operations for Inpatient and Outpatient services via a decentralized or centralized pharmacy. Age-specific dosing and pharmacokinetic dosing, renal and/or hepatic impairment dosing, pregnancy and/or lactation drug contraindications, drug/drug interactions, drug/food interactions, drug/disease interactions, IV compatibility, collaborative Drug Therapy Agreement, pharmacist prescribing by protocol, prescriber-assigned dosing, and by prescriber-assigned consults. Computer order entry and verification of patient profiles, sterile products, IV admixtures, parenteral nutrition solutions, compounds and chemotherapy, aseptic technique and Infection Control standards, controlled substances management, code Blue participation, teaching and/or orientation of patients/families and healthcare providers, quality assurance studies and performance improvement, Drug Usage Evaluation, Target Drug Programs, Adverse Drug Reaction investigations, Medication Use Variance monitoring and intervention
