Job Summary

The Primary Care Physician provides direct medical care to participants enrolled in a Program of All-Inclusive Care for the Elderly (PACE). This role works closely with nurse practitioners, the Medical Director, and the interdisciplinary team (IDT) to ensure coordinated, comprehensive, and person-centered care. The physician also plays a vital role in 24/7 medical coverage through a shared on-call system.

Key Responsibilities

Clinical Care

  • Supervise and collaborate with Certified Registered Nurse Practitioners (CRNPs) for participant care
  • Perform comprehensive history and physical exams for new participants, including establishing care goals and end-of-life preferences
  • Conduct regular participant assessments (annual, semi-annual, or as conditions change)
  • Diagnose and treat acute illnesses in alignment with participants' POLST and advance directives
  • Engage participants and families in ongoing conversations about end-of-life care
  • Participate in IDT meetings to review participant status and develop care plans
  • Communicate clearly and regularly with the IDT regarding participants' medical, functional, and psychosocial status
  • Discuss treatment options with participants and/or their legal representatives in alignment with care goals
  • Oversee the use of medical specialists and coordinate inpatient care as needed

Hospital & Skilled Nursing Facility (SNF) Coordination

  • Manage participant care when admitted to contracted or non-contracted hospitals or skilled nursing facilities
  • Serve as a liaison with facility staff and ensure continuity of care
  • Provide regular updates to the IDT on participant progress and hospitalization status
  • Lead timely discharge planning and appropriate use of specialty consults

Call Coverage

  • Participate in rotating night and weekend call schedules to provide 24-hour medical coverage
  • Ensure appropriate triage and response to participant concerns after-hours

Documentation & Quality

  • Complete timely and accurate documentation in the medical record
  • Notify the Medical Director of unusual or urgent events as needed
  • Actively contribute to Quality Assessment and Performance Improvement (QAPI) activities
  • Follow policies and procedures related to medical documentation and compliance standards

Interdisciplinary Team Involvement

  • Attend and participate in IDT meetings, care conferences, and emergency meetings
  • Share updates regarding participant changes and collaborate on care planning
  • Provide in-service education and clinical guidance to staff as needed
  • Participate in shared decision-making across non-clinical and clinical team matters
  • Be readily available for consults and support care transitions

Working Conditions

  • Primary work location is at adult day health centers; must also travel to participant homes, hospitals, and skilled nursing facilities as needed
  • Must be comfortable working in clinical settings that may involve exposure to bodily fluids, communicable diseases, and strong odors
  • Required to use Universal Precautions and adhere to infection control protocols at all times
