Purpose

Step by Step Supportive Services (SxS) empowers adults who deal with challenging mental health experiences to lead enriching, independent lives through a supportive environment and community connection. Responsibilities

  • Take on responsibility as directed with one of the core functions of the clinical leadership team, including Admissions, Operations, Therapy, Outreach, and Program Quality Assurance.
  • Independently and effectively perform all duties of one of the core functions as requested including providing progress updates to Senior Leadership Team (SLT) when core function primary manager is unavailable due to paid time off, maternity leave, leave of absence, temporary reassignment, during staffing/hiring transitions, or if there is no core function primary manager.
  • Identify and share core function process or program improvement opportunities when applicable to core function primary manager or to SLT in the absence of a core function primary manager.
  • Provide direct clinical supervision to clinicians, interns, mental health workers, and/or per diem team members as needed.
  • Assist with developing, implementing, and directing holistic wellness programming.
  • Provide up to 20 hours of clinical care support weekly, which includes individual sessions, family coaching, care coordination, team meetings, and travel time. Responsibilities include the following:
  • Develop and update Individual Support Plan (ISP) with clients based on service type and governing regulations.
  • Provide support with symptom management.
  • Provide support with developing life skills such as establishing daily routines, maintaining a clean living space, completing laundry and other tasks for independent living.
  • Provide financial management support including creating, implementing, and reviewing an individualized budget for each assigned client.
  • Assist clients in developing structure through productive activity- clients should maintain a minimum of 20 hours a week of structured activity. Assist clients in finding appropriate employment and/or volunteer opportunities.
  • Provide support with weekly healthy meal planning and grocery shopping.
  • Provide support with overall health and wellness goals.
  • Assist clients with developing executive functioning skills such as managing a schedule, apartment organization and developing strategies to increase task/goal follow through.
  • Support clients with time management including maintaining their own schedule of appointments, classes, work, etc.
  • Ensure clients keep their apartments organized, clean and well maintained. Routinely visit clients in his/her/their apt as part of case management or other SxS program activities/opportunities. Advocate with and for clients' rights to timely and thorough repairs of apartment as needed.
  • Utilize a community integration approach to empower clients to play an active role in their recovery, develop skills for autonomous living and build community connections.
  • Coordinate services with client's outpatient treatment team to achieve holistic care.
  • Advocate for client in obtaining and maintaining additional outside benefits.
  • General daily support as needed.
  • Provide intensive support and advocacy as needed.
  • Other services on an individualized basis.
  • Plan and facilitate psychoeducational, skill building, and/or processing groups based on agency needs (for example, CBT, DBT, expressive therapies, etc.).
  • Provide timely, thorough, and accurate clinical documentation of services provided by or before the end of every shift.
  • Submit accurate timecards of hours worked to the SxS bookkeeper by close of business every other Friday, Saturday or Sunday (dependent on assigned workweek).
  • Properly prepare for and actively participate in weekly SxS Team Meetings, weekly individual supervision, periodic SxS Family Town Hall Meetings, and additional SxS client/family functions as needed.
  • Be a proficient user of SxS's electronic medical records system (BlueStep).
  • Acquire and renew before expiration all SxS required certifications including HIPAA, Crisis Prevention, First Aid, Blood Borne Pathogens, etc.)
  • Coordinate and facilitate the weekday evenings SxS M.E.A.L.S. program working closely with a host client to plan, prepare and provide nutritious weekday evening dinner meal for SxS community peers, actively share in the meal, facilitate appropriate dinner conversation, foster peer community connection and support, and coordinate post-dinner clean-up with participants.
  • Drive the SxS van with clients or facilitate other transportation when required to facilitate clients' medical or other care team appointments, classes, gym trips, etc.
  • Remain available, accessible and prepared to respond during assigned on-call rotation weeks.
  • Create and maintain a safe and supportive atmosphere by following all SxS program policies, protocols, and procedures.
  • Ensure effective communication of priority information to all necessary staff and shifts.
  • Respond appropriately to emergencies including contacting appropriate staff and supervisor(s), interacting with police, fire, and medical personnel, and assisting clients directly as needed and appropriate.
  • Respond to all emails and phone calls within 24 hours.
  • Ensure the health, safety, and well-being of each client.
  • Foster and support the fundamentals of a caring community within the SxS agency with clients and fellow SxS team members.
  • Report any human rights violations or concerns to the SxS Human Rights Officer.
  • Up to 5% of time may be on other tasks and duties as assigned.

Qualifications

  • Master's Degree in social work field
  • Independent Clinical Licensure (LICSW)
  • Minimum of three years of experience as a Clinician at Step by Step Supportive Services or a similar mental health service agency
  • Has a track record of excelling in all Clinician job responsibilities and demonstrates leadership within the SxS team or at a similar mental health service agency
  • Computer literate
  • Ability to consistently meet all necessary background and credentialing requirements
  • Valid driver's license; must have reliable transportation to and from work
  • Covid-19, flu and measles vaccination records in-line with the CDCs current recommendations as to best practice

Compensation

  • $80,000-87,000 based on experience
