Our workforce is the heartbeat meeting the needs of our local communities, don't wait, join our team!Position Summary:The ACT Team Registered Nurse functions as an integral part of the Assertive Community Treatment Team to provide intensive, integrated, and recovery-oriented treatment for adults with serious mental illness and is a high-risk consumer of mental health resources. The ACT Team Registered Nurse works collaboratively with medical providers and pharmacies to aid in medication management; engages in health promotion, education, training to other team members; andassistsconsumers with understanding medications prescribed and the importance of taking medication. A person in this position must exercise tact, judgement, and confidentiality in contact with the public, clients, and families, as well as fellow associates of 4C Health. This position reports directly to the Director of Nursing.Position Responsibilities include but are not limited to:
  • Work collaboratively as part of the ACT Team in order to provide comprehensive care that meets the individual needs of each client enrolled in ACT.
  • Assess client health daily and respond appropriately to any noted concerns (i.e., medication side effects, withdrawal symptoms, general health changes, etc.)
  • Document timely, accurately, and in accordance with 4C Health policies utilizing current systems and practices.
  • Monitor client use of medications, changes in medications, and assess for efficacy, safety, and/or side effects of medications to assist client in developing medication autonomy. Reconcile client medication with pharmacy, client chart and pharmacy.
  • Knowledge, understanding, and experience providing mental health and substance use treatment, including experience conducting mental health evaluations, familiarity with psychiatric medications, and awareness of medication assisted treatment (MAT) practices.
  • Collaborate with all providers involved with client's health care and provide feedback to providers as needed.
  • Link clients to appropriate healthcare providers or specialists and follow-up with all referrals and recommendations.
  • Provide health and wellness education to the client, the client's supports, and other ACT Team staff members.
  • Participate in the formal training and development process with administrative staff, Client Care Specialists and Skills Trainers, as well as ongoing professional development and continuing education (including but not limited to, symptom management and medication side effects)
  • Attend daily team meetings, individual supervision, and group supervision as scheduled.
  • Coordinate care between ACT clients and ACT treatment team.
  • Provide client centered nursing interventions.
  • Support client participation in the community.
  • Initiate and maintain relationships with community resources, outside human service agencies, and other stakeholders as needed.
  • Commit to ensuring fidelity to the ACT model and ongoing quality improvement efforts.
  • Crisis Intervention: Participate in crisis intervention and de-escalation strategies and provide support and resources for clients and treatment team members.
  • Local travel within the service delivery area is required as many ACT services are provided in community-based setting.

  • Eligible for $150/month ($1,800 annually) in student loan repayment,available at hire
  • Earn$750for every person you refer who is hired and remains employed for 6 monthsand receive an additional $500every year as long as you both remain employed

Education and Experience:
  • Licensed as a RN in the state of Indiana, current and valid.
