Overview Under the direct supervision of the Nutrition Program Coordinator (Program Manager), performs the essential duties of an Outreach Worker. Receives, logs, screens and processes request for assistance. Conducts home visits to assess client needs and eligibility for Home Delivery Meal Services. Develop case plans and facilitate services for clients with outside social service agencies and make appropriate referrals. Monitor client's progress and conduct regular reassessments of client's eligibility status. Collects, complies and submits required reports. Provide a full array of direct social services on a one-to-one basis with the elderly community. Identify isolated, low income, minority elderly to determine eligibility for benefits and services they might otherwise not access. Display good working knowledge of community resources and social service agencies. Respond to crisis situations and aid seniors in need. Communicate with route drivers on any significant changes in client status. Work closely with Nutrition Program staff to coordinate an efficient delivery route system. Adhere to home delivered Meal program standards governing operations. Responsibilities • Receives, logs, screens, and processes daily Client Referrals from the community, hospitals, Home Health Agencies, Social Workers, etc. prior to conducting a home visit to determine the appropriate service need for the individual • Must have the ability to communicate with both English and Spanish speaking elderly clients in person and on the telephone to determine their eligibility for the Home Delivery Meal Program and other community services • Possess good interviewing skills and ability to work with hearing and vision impaired clients • Ability to use good judgement in scoring clients on a standardized questionnaire (Form 2060) to ascertain their Activities of Daily Living (AOL) and Independent Activities of Daily Living (IAOL) levels as required by the Texas Department of Health and Human Services • Develop a case pain for each Home Delivered Meal client as needed • Coordinate direct case management services including but not limited to assessment (ADL/IADL's), benefits counseling, referrals and follow up to appropriate agencies/organizations, personal assistance by filling out forms (housing, food stamps, transportation, etc.) reading and writing letters, home visits, telephone reassurance, emergency transportation, escort and advocacy • Collect, compile data and complete technical reports and correspondence • Generate monthly program performance and benefits counseling reports and assists in problem solving techniques and processes • Comply with program requirements for proper documentation of all client contracts, program reports, memos, and other general correspondence • Provide ongoing communication with route drivers and the Nutrition Program Coordinator regarding any substantial changes in health and mental state or living conditions of the Home Delivered meal clients • Respond immediately to emergency situations of clients by conducting home visits and providing assistance for any needs such as calling for medical assistance, food, housing or income assistance • Demonstrate good working knowledge of State and Federal standards pertaining to all phases of the Home Delivered Meal Program Position Type and Typical Hours of Work • Non-Exempt - Part-Time • Flexibility to work evenings, weekends, and holidays is a schedule requirement • In the event of an emergency, employees are required to work to provide for the safety and well-being of the public, including the delivery and restoration of vital services. Minimum Qualifications • Associate's degree or two (2) year technical certificate • Two (2) years of experience Licenses and Certifications Required • A valid driver's license is required; successful out-of-state candidates must be able to obtain a valid Texas driver's license within 90 days of hire • Food Handler Permit • Basic First Aid • CPR • Must pass pre-employment and monthly background checks as required by the Home Delivered Meal Program Grant Employment Testing Employment is contingent on passing any post-offer pre-employment screening as listed below: • Criminal Background Check: Yes • Motor Vehicle Record Check: Yes • Drug Screening: Yes • Physical Exam: Yes Basis of Rating A recruitment consultant will evaluate all applications against the posted qualifications. The city may also conduct additional skill assessment tests, in addition to the panel interview.
