Ontario Center is hiring a Part-Time Social Services Assistant in Canandaigua, NY! Duties:Assist in the planning and implementation of programs to meet residents' social and emotional needs. Support residents and families through the admission process and throughout their stay. Document services provided in compliance with regulatory standards. Collaborate with nursing and other facility departments to enhance quality of life for residents. Help develop individualized care plans to address resident needs. Support discharge planning and community resource coordination. Foster a welcoming, respectful, and resident-centered environment. Requirements:Associate's Degree in Social Work, Human Services, or related field (Bachelor's preferred)Experience in long-term care or healthcare setting preferred.
