Overview: We are excited to invite a dedicated Hospice Patient Care Coordinator to join our compassionate team. You will play a pivotal role in overseeing our high-performing hospice program, ensuring that quality patient care is delivered consistently. In this position, you will report to the Executive Director, Administrator, or Senior Patient Care Manager. Your responsibilities will encompass managing patient care activities, educating and developing staff, and ensuring compliance with company policies and legal regulations.
- Manage rescheduled, declined, missed, and reassigned visit requests by assessing priorities and staffing needs using your clinical expertise.
- Respond to phone referrals and inquiries with professionalism and compassion, effectively triaging medical concerns within your scope.
- Prepare for, participate in, and facilitate the Interdisciplinary Group (IDG) meetings, presiding over discussions, coordinating minutes, and reviewing patient charts for any changes in condition or care plans while actively soliciting input from care team members.
About You:
- Minimum three years of direct patient care experience and two years of experience in a hospice or home-based care setting.
- A graduate of an accredited nursing school, with a preference for a bachelor's degree in nursing.
- Current Registered Nurse licensure in your state of residence.
- Strong understanding of accepted hospice practices and principles, including regulations and best practices (e.g., Medicare, Medicaid, JCAHO, ACHC), along with a compassionate understanding of terminally ill patients and their families.
- Ability to address the unique needs of hospice patients and their families, advocating effectively across diverse socioeconomic backgrounds.
