JOB DEFINITION:

The Social Worker functions as a member of an interdisciplinary team to provide assessment and referral to the jail diversion programs through Nassau County. Assessments are provided to severely mentally ill adults with co-occurring substance abuse disorders and/or medically ill individuals who have current legal issues as the result of their illnesses. Advocates for and supports the client, engages with community agencies/health care providers and others on his/ her behalf to ensure access to appropriate jail diversion programs. Provides clinical support to the Team by providing consultation, education, information around psychosocial and/or substance abuse conditions, interventions, resources to maintain focus on outcomes and best practices. The Social Worker must be available on an as-needed basis 24 hours a day, 7 days a week.

EDUCATIONAL REQUIREMENTS:

Master's Degree in one of the following fields: Social Work or Mental Health Counseling. License required.

EXPERIENCE REQUIRED:

  • One (1) year of related human services experience required in providing direct services to mentally, developmentally or other disabled clients in order to link them to a broad range of services essential to successfully living in the community.
  • Working knowledge of computer software and electronic health record systems.
  • Demonstrated competency in written, verbal and computational skills to present and document records in accordance with program standards.
  • Experienced in and demonstrated comprehensive understanding and working knowledge of the interdisciplinary planning process and the developmental treatment model.
  • Knowledge of Medicaid, Social Security and other entitlements preferred.
  • Excellent interpersonal skills required.
  • Must have the ability and willingness to regularly travel, in some instances with clients in Agency vehicles, to many locations using various modes of reliable and safe transportation. If working in Nassau or Suffolk Counties, a valid Tristate (New York, New Jersey or Connecticut) driver's license is required; and you must have a current and valid driver's license on file with CNGCS' Human Resources and be designated and remain in good standing as a CNGCS authorized driver
  • Willingness to travel to the Nassau County Jail and courthouse.

DUTIES AND RESPONSIBILITIES:

  • Conducts initial assessments of assigned clients to document eligibility to any of the jail diversion programs in Nassau County.
  • Participates in the development and documentation of client centered comprehensive, integrated, interdisciplinary care plan in consultation with other team members to ensure focus on desired outcomes.
  • Completes referrals for all needed services as outlined by the Court
  • Provides supervised toxicology screens as requested by the Court or as needed as part of the assessment.
  • Maintains effective communications with clients, court officials, primary care physicians, substance abuse and mental healthcare providers, family, collateral resources and other Agency staff on behalf of clients.
  • Maintains documents, records, statistics, and other related reports in an organized, timely and accurate manner as per policy and procedure.
  • Prepare written reports of assessment outcomes and recommendations that are shared with the multidisciplinary team.
  • Works as part of a multidisciplinary team, attends and participates in team meetings to provide input/feedback around psychosocial conditions/comorbidities to review client status, update plans and goals, review outcomes to further program goals.
  • Compiles weekly monitoring reports and submits all necessary documentation according to applicable agency policy and/ or HIPAA standards for jail diversion cases prior to when they appear in court.
  • Schedules, manages and attends regular court appearances in order to advocate for persons served and report adherence to terms of court orders; documents all judicial outcomes and reports them to the Team and team leadership as needed.
  • Acts as a resources/consultant to all team members on psychosocial and/or substance abuse issues and resources.
  • Provides telephonic as well as face to face outreach, engagement and service planning in the field.
  • Acts as a linkage to community services including medical, behavioral, residential, entitlement and any other needed services per interdisciplinary care plan.
  • Monitors overall service delivery to clients to ensure coordination and continuity; advocates with service providers/resources as needed.
  • Maintain paper charts as well as updating information into the UCMS database.
  • Provides crisis intervention and follow-up on an as-needed basis 24 hours a day, 7 days a week.
  • Participate in training, educational and committee projects as requested.
  • Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress
  • Complete QI Training during onboarding and participate in QI refresher training as needed.
