Benefits: 401(k) Dental insurance Health insurance Paid time off Vision insurance COMMUNITY HEALTH WORKER Overview of Role: Under the direction of the Director of Community Health and Wellness, the Community Health Workers (CHWs) are frontline public health workers who serve as a bridge between healthcare providers and the community to promote health, prevent disease, and help manage chronic conditions. They work to improve the quality and cultural competence of service delivery by building individual and community capacity through various activities such as outreach, community education, informal counseling, social support, and advocacy. Position Summary: The Community Health Worker (CHW) will play a critical role in improving community health by engaging with individuals and families to provide support, education, and resources that promote health and well-being. The CHW will work closely with healthcare providers, social services, and other community-based organizations to ensure that individuals receive comprehensive and coordinated care. This role requires a compassionate and dedicated individual with strong communication skills and a deep understanding of the community's needs. Outreach and Community Engagement: Conduct outreach activities to identify individuals and families who may benefit from health and social services. Build trusting relationships with community members to encourage their participation in health programs and services. Conduct door-to-door outreach, attend community gatherings, and participate in local events to increase visibility and accessibility. Health Education and Promotion: Provide culturally appropriate health education and information to individuals and groups. Organize and facilitate community-based health education workshops and events. Distribute educational materials and resources to community members. Care Coordination and Advocacy: Assist individuals in accessing healthcare services, including scheduling appointments, providing transportation assistance, and navigating the healthcare system. Advocate on behalf of individuals and families to ensure they receive appropriate services and support. Support and Counseling: Offer informal counseling and social support to individuals dealing with health-related issues. Help individuals develop self-management skills to improve their health outcomes. Data Collection and Reporting: Maintain accurate and confidential records of interactions with community members. Collect and report data on community health needs and program outcomes to inform service planning and improvement. Prepare regular reports for the Director of Community Health and Wellness. Collaboration and Partnership: Work closely with local healthcare providers, non-profits, and community organizations to coordinate services and programs. Participate in community health coalitions and task forces to address broader health issues. Foster partnerships with local businesses, schools, and faith-based organizations to expand outreach efforts. Requirements: High school diploma or equivalent; additional education or training in health or social services is preferred. Certification as a Community Health Worker (CHW) or willingness to obtain certification within a specified timeframe. Prior experience in community health, social services, or a related field is desirable. Success working with communities, with the ability to cultivate existing relationships and establish new relationships. Strong knowledge of the community and its resources. Excellent interpersonal and communication skills. Ability to work independently and as part of a team. Proficiency in relevant languages spoken by the community is an asset. Valid driver's license and reliable transportation is required. Competent computer skills (proficiency in MS Office products is a must) Unwavering commitment to the organization, neighborhood revitalization and transformation. Strong work ethic with an ability to work independently or as a team. Problem solving skills when resolving issues and conflicts. Relationship Building: Cultivating and maintaining positive relationships with Bonton community members, local leaders, organizations, and stakeholders to build trust and rapport. Needs Assessment: Conducting assessments to identify the specific needs, concerns, and aspirations of the Bonton community, and using this information to inform organizational planning and decision-making. Collaboration: Initiating and managing collaborative projects and partnerships between the organization and Bonton community entities, promoting a sense of shared responsibility and mutual benefit. Communication Skills: Strong verbal and written communication skills. Ability to convey information clearly and effectively to individuals and groups. Problem-Solving and Advocacy: Strong problem-solving skills and the ability to advocate for the needs of individuals and families. Ability to navigate complex healthcare and social service systems. Team Collaboration: Ability to work collaboratively with healthcare providers, social services, and community-based organizations.
