Job Description & Requirements

- Assess patient/family needs and barriers affecting treatment or discharge planning.
- Collaborate with interdisciplinary teams and physicians to develop individualized care plans and safe discharge plans.
- Perform utilization reviews and ensure compliance with medical necessity criteria and payor guidelines.
- Educate patients and families on available resources and facilitate referrals to community agencies.
- Manage insurance certifications, payor communications, and documentation for reimbursement.
- Identify and eliminate unnecessary tests, procedures, or appointments to support cost-effective care.
- Maintain accurate and timely medical record documentation.
- Serve as a liaison between the facility, patients, families, and outside agencies.
- Participate in team and family conferences, scheduled meetings, and facility-wide problem-solving.
