Assisted Outpatient Treatment Case Manager The Assisted Outpatient Treatment (AOT) Team is an innovative, wrap-around service that provides intensive support to participants. We are seeking a dedicated and compassionate Case Manager to join our team. In this role, you will provide case management and care coordination services for individuals under court-ordered outpatient treatment, ensuring compliance with treatment plans while promoting stability and independence in the community. This includes conducting regular outreach and home visits to assess clients' progress and address any barriers to treatment adherence, coordinating mental health, medical, housing, and social services to support clients' recovery and stability, and maintaining accurate and timely documentation. You will be part of a skilled, multi-disciplinary treatment team consisting of nurses, therapists, case managers, peers, and prescribers.Responsibilities Primary Job Functions:Clinical Responsibilities:Demonstrates the ability to assess the consumer's needs utilizing the SUN-RIdentifies goals and plans for appropriate services documented in ISPProvides appropriate linkage to community resourcesMonitors consumers to secure placement in the least restrictive environmentProvides ongoing monitoring of consumers' progress and needs as assessed in the SUN-RAdvocates on behalf of the consumer to secure community resourcesEffectively collaborates with the treatment team members and outside sources in the delivery ofWorks closely with treatment team and ensures that the team is aware of anyAssists in de-escalating crisis situations with consumers and/or their familiesEffectively demonstrates the application of adult specific competenciesProvides outreach services to consumers and familiesProvides training of basic living skills to improve consumers quality of lifeAssists consumers with transportation needs to psychiatric appointmentsMakes multiple contact per week with consumers, depending on consumer needsPromptly engages consumers in services following referralMonitors for compliance with outpatient commitment orders and reports and non-complianceClinical responsibilities a. Assesses consumers' financial resources to determine the need to apply for benefits.Assists consumers in completing applications to secure benefits.Assists consumers in obtaining documentation required for applications to Social Security Administration.Advocates on behalf of consumers with the Social Security Administration office.Effectively utilizes the SOAR system.Tracks consumers' statuses that have applied for benefits.Ensures the Finance and Accounting department and treatment team is notified when benefits are obtained.Ensures matching of appropriate services for multiple consumers in case load.Seeks clinical supervision and consultation as neededParticipates in weekly staff meetings to address concerns regarding consumer with supervisor and other members of the treatment teamAccepts and employs suggestions for improvementActively works to enhance case management skillsDocuments in a timely manner per AltaPointe standards, within 24 hours of provision of serviceDocuments appropriate, complete, and comprehensive SUN-R assessmentsDocuments measurable and specific goals in the individualized service planDocuments in a clear and concise manner, consumer problems, ISP goals addressed and services providedIncludes problem, ISP/Treatment plan goal, intervention provided, consumer responses, and plans for continued services in each progress noteDocuments case disposition, case closings, and follow-upDocuments legibly.Effectively manages caseload based on consumers' needs.Meets AltaPointe productivity standards.Courteous and respectful towards consumers, visitors and co-workers a. Actively participates in Performance Improvement activities.Actively participates in AltaPointe committees as required.Completes assigned tasks in a timely manner.Follows AltaPointe policies and procedures.Attends required in-service trainings and other workshops.Qualifications Bachelor's degree in a mental health discipline; certification of case management training by Alabama State Department of Mental Health and Mental Retardation; ability to stay organized and multi-task a workload with multiple consumers;Knowledge in adult specialty, outlined below, to be attained within six months of employment:Knowledge of psychiatric disorders in the adult populationKnowledge of cultural diversityKnowledge of behavior management, crisis intervention and interventions common to acute psychotic consumers, as well as, to non-violent crisis intervention practiceKnowledge of psychiatric rehabilitation principlesKnowledge of available community resourcesKnowledge of educational methods appropriate to adults with severe and persistent psychiatric disordersKnowledge of the social security system in order to obtain benefitsKnowledge of the SOAR system
