The Executive Director of Population Health provides leadership, strategic direction, and operational execution to the AdventHealth Physician Network (AHPN). Population Health is defined as A model that delivers high quality, people-centered care through highly efficient and effective processes at every venue of care throughout a highly aligned, meaningfully connected ecosystem throughout a patient's lifetime. Under supervision of the SVP of Population Health, the Executive Director coaches, mentors and leads Directors, Managers, and staff in the daily operations of assigned areas. In partnership with the AHPN and other Population Health teams, this role engages with stakeholders in the planning, design, execution and support of Provider Engagement & Performance Quality & Network Design, Chronic Care Management, Network Operations and Network Development, ensuring effective support of AdventHealth policies. This position is responsible for the implementation of structures, protocols and processes to achieve the strategic vision for performance in the network, operational delivery of established structures, protocols and processes that drive the performance of the network in AHPN value-based arrangements defined as lowering total cost of care below contracted targets and exceeding quality and experience requirements. This leadership role drives performance in quality, utilization, and service delivery expectations which include connected, convenient and complete care. The Executive Director is responsible for driving and supporting the organization's Population Health employed and community provider's through contracting and strategic alignment, market executive engagement, network adequacy and overall network integrity for value-based Direct to Employer, Commercial and Government contracts. Accountable for creation of, deployment of and operational execution of protocols and systems including but not limited to analytics, care management, meaningful interoperability, communications, education, and CDI. Adheres to the AdventHealth Corporate Compliance Plans and to all rules and regulations of applicable local, state, and federal agencies and regulatory bodies. Actively participates in outstanding customer service and accepts responsibility for maintaining relationships that are equally respectful to all.
