This position is responsible for coordinating patient care assignments, answering the phone and other administrative duties to support the Home Health Department. Job Functions Coordinates and manages patient care assignments for all Home Health professionals and paraprofessionals. Communicates and coordinates referrals with Intake Staff. Serves as a liaison between clients and staff as well as staff, physicians, pharmacy, lab, supplies and/or messages as appropriate. Provides customer service to internal and external customers by answering inquiries or referring to the appropriate staff personnel. Participates in on-call staffing rotation according to department needs. Assists in record keeping. Performs other duties as assigned.
