Functions as a key member of the healthcare team using sound nursing judgment. Participates in planning, implementing, and evaluating patient care through focused assessments. Seeks guidance from an RN or healthcare team when: Patient needs exceed LPN scope of practice. Care needs surpass LPNs knowledge or ability. Patient condition deteriorates or changes significantly. Obtains supervision when performing new or unfamiliar procedures. Adheres to UPMC policies, LPN Practice Guidelines, and State Practice Act. Provides IV therapy within LPN scope of practice. Uses Relationship-Based Care to create a healing, patient-centered environment. Develops daily care plans and goals in partnership with patients. Ensures thorough documentation to promote caregiver communication. Aligns daily work with professional, unit, and system goals for patient safety and quality.
