Access. Health Anderson is a program to assist qualifying low-income adults without health insurance in Anderson County. Access. Health brings together a network of healthcare providers and community agencies to ensure that those in underserved communities have access to quality of care in the right place at the right time. The Community Health Worker (CHW) provides healthcare navigation services to enrolled patients, coordinates medical home placements and first appointments, and collaborates with a multidisciplinary team to address the social determinants of health. The CHW provides health coaching and advocacy to improve healthcare utilization and assists enrollees with applications for healthcare payor options, prescription assistance and/or other benefit programs as appropriate. The CHW will be expected to attend appointments and make home visits as required to advocate for the enrollee and to connect them to needed resources.
