Isaacs Neighborhood Center (Isaacs Center) are well-established Manhattan community-based agencies that support services to approximately 30,000 New Yorkers. We operate at community centers located in the neighborhoods of East Harlem and Yorkville, and seek to deliver programs that are impactful, innovative, and intergenerational.Program Description: Goddard Riverside's Corner House provides affordable, permanent supportive housing for 34 previously homeless adults many of whom have mental illness, substance use, and other co-occurring disorders. On-site services include case management, entitlement assistance, medical and psychiatric care, substance use counseling, group activities, recreation, and 24 hour/7 day per week front desk security.Position Summary: To deliver effective case management services using a strengths-based, client-centered approach to formerly homeless adults diagnosed with a severe and persistent mental illness in order to maintain permanent housing. The Senior Case Manager will provide administrative support while carrying a caseload. The Senior Case Manager will supervise, guide, oversee employee performance, and take control of implementing improvements on a daily basis. As a Senior Case Manager, you must demonstrate Integrity, flexibility, problem-solving, direct moving projects along faster and improve efficiency. Senior Case Manager must demonstrate strong decision-making skills and clear communication.Schedule: Monday-Friday 9:00am-5:00pmSalary Range: $53,000-$60,000Candidates must have the required qualifications to be considered for the position. The salaryoffer will be basedonthe candidate's number of years experience in the exact orcomparableposition and additional preferred qualifications will be taken into account.Roles, Responsibilities, and Essential DutiesRoles, Responsibilities and Essential Duties1) Client serviceProvide case management to a group of assigned clients; identify short and long-term treatment goals and help clients achieve them, including linkages to community programs such as medical, psychiatric and drug treatment services.Assist clients in receiving and maintaining entitlementsAssist clients in understanding and complying with building rules to maintain residencyMaintain case records with documentation of services provided and outcomesResponsible for case notes and service plans using an electronic databaseCoordinate meetings with clients and building managementCoordinate and track medical appointments between tenants' and various medical providersAssist in taking clients on outdoor events when low on staff2) Administrative DutiesAssist the Program Director with training new social service staffComplete case record audits monthly for the ProgramParticipate in tenant intake/admission meetingsSupport the Program Director in serving as a Liaison to other community services; attend meetings and develop new linkages in the absence of the PDServe as senior staff member in Director's absenceTrack and maintain records for staff trainings, and schedule staff needed.Coordination of programs and community referralsProvides culturally responsive crisis-intervention, advocacy, and supportive counseling services to clients and supervises the case management team.Participates in ongoing training and educational opportunities; provides educational resources to staff and volunteer advocacyDevelops ongoing referral relationships with partner organizations and within the health, legal , and social services systems3) Participate as Corner House team memberAttend staff meetings, case conferences, training programs and supervision meetingsAttend Incident Review Committee meetings in the absence of the Program DirectorCollaborate to maintain an active resource directoryConduct apartment inspections in conjunction with building management4) Other duties as required in accordance with agency polices and funding source requirementsQualifications/Educational RequirementsBachelor's Degree required in related field.
