Job Description :

HANAC, Inc. – originally founded by George Douris as the Hellenic American Neighborhood Action Committee – is a New York City-based multi-faceted social services organization. The organization was founded in 1972 to serve the needs of vulnerable populations throughout New York City.

The Case Planner will carry caseloads of 12 families and provide individual and family counseling to at-risk families. Case Planners will assess, evaluate, and monitor safety and risk, conduct home visits, intake assessments, provide case management, participate in family team conferences, ensure data entry into Promis and Connections, prepare progress notes, FASP's, and Closing Plan Amendments in a timely manner, and maintain compliance with agency protocols and contract mandates.

Salary: $53,000 per year (commensurate with experience, degree, and language skills).

Work Schedule: Varies; Two late nights required from 12 pm to 8 pm.

FLSA Status: Non Exempt

Work location address: 23-16 30th Ave, Second Floor, Astoria NY, 11102 (Must be available to work on-site)

Bilingual skills: Bilingual is a plus.

Major Functions/Accountabilities:

  • Maintain a caseload of 12 families.
  • Conduct weekly family and individual sessions to address service outcomes. Maintain 2-3 contacts per month with each child under 18 and assess safety and risk.
  • Complete assessments, including FASP's, Psychosocial Assessments, and Plan Amendments, promptly.
  • Complete progress notes within 24 hours of contact.
  • Develop service plans collaboratively with families.
  • Engage families from intake to case closure to improve outcomes.
  • Attend weekly supervision meetings to discuss at least 3 cases and high-risk cases as needed.
  • Collect report cards, attendance, and medical data for children.
  • Refer families to community services when appropriate.
  • Attend required training and meetings, including community events and coalitions.
  • Keep supervisor informed of high-risk situations.

Qualifications:

  • Bachelor's Degree in Human Services or related field.
  • At least 2 years of experience working with families and children; familiarity with NYC Child Welfare systems is a plus; computer literacy required.
  • Excellent communication and organizational skills.
  • Ability to work independently and as part of a team, maintaining professionalism and confidentiality.
  • Ability to meet deadlines and follow instructions.
