Description
Promote permanency for children and youth in foster care by facilitating family participation and providing an independent perspective during service plan reviews. Support and promote strengths-based casework through the Family Team Conference model.
Responsibilities
- Convene and facilitate family team conferences, including planning service reviews, goal changes, placement changes, and therapeutic care transitions.
- Submit Family Team Conference requests to ACS and follow up with staff as needed.
- Maintain communication with ACS and program staff according to conference schedules.
- Promote and monitor permanency planning, ensuring compliance with federally mandated timeframes.
- Attend ACS transitional conferences and create/update Genograms.
- Prepare and distribute monthly reports on permanency and transitional conferences.
- Participate in making quality decisions regarding child safety, well-being, and permanency, and make recommendations to case planners and service providers.
- Collaborate with community representatives to ensure participation in conferences.
- Complete administrative tasks, including documentation and data entry in CONNECTIONS.
- Facilitate up to three conferences per day.
- Ensure compliance with agency policies, procedures, and regulations, including HIPAA.
- Perform other duties as assigned by supervisor.
Qualifications
- Master of Social Work + 2 years foster care experience OR Bachelor's degree + 5 years foster care experience.
- Exceptional mediation skills for group facilitation.
- Ability to summarize solutions and facilitate consensus.
- Flexible work schedule, including evenings and weekends.
- Ability to travel within NYC and Long Island.
- Ability to interpret policies and write routine reports.
- Strong organizational, interpersonal, verbal, and written communication skills.
- Proficiency in MS Office and CONNECTIONS.
Other Skills and Requirements
- Proficient in Microsoft Word, Excel, and CONNECTIONS.
- Travel within NYC boroughs and Long Island as needed.

- Experience working with diverse populations, including children and families with various backgrounds and challenges.
- Ability to work with individuals identifying as LGBTQ+ or exploring their identity.
- Willingness to work varying schedules, including weekends.
