PRN Discharge Planner Valle Vista Hospital

Since 1983, Valle Vista Health System, a 132 bed facility located in Greenwood, Indiana, has been providing high-quality behavioral health care to the community. We offer care for a broad range of psychiatric and addiction disorders. We provide care to adults, and to patients as young as age five. Our programs include inpatient, outpatient and medication-assisted treatment for adults, and residential and inpatient treatment for children and adolescents. These programs are designed to help adults, children and teens learn the necessary skills to regain confidence, heal and successfully function at home, work, school and in their communities.

Job Summary The Discharge Planner is responsible for the majority of discharges on units within VVHS. The Discharge Planner completes individualized discharge and safety plans. The Discharge Planner provides patients with a review of treatment options beyond VVHS. The Discharge Planner represents VVHS in coordination of treatment follow up for patients within the community. The Discharge Planner works closely with other members of the social services team from the time of intake to plan for an appropriate discharge plan.

Qualifications: Education/Licensure: Bachelor's degree from an accredited college or university, in social work or a clinical related mental health field. Experience: Previous experience in a psychiatric health care facility, with direct experience in case management, care coordination and interdisciplinary team staffing preferred. Additional requirements: Required to work flexible hours and weekends.
