Compensation is dependent on skills and experience.Position Summary: The Patient Care Navigator is responsible for collaborating with medical and behavioral health staff to support asylum seekers within HealthPoint's service area. This role works closely with clinics and expanded care teams to enhance access to care, address barriers faced by migrant communities, and support efforts to prevent and limit the spread of communicable diseases. Compensation is dependent on skills and experience.Your contribution to the team includes:
  • Ability to problem solve using critical thinking skills, judgment abilities, visionary capabilities, and creativity as it relates to supporting asylum seekers in South King County.
  • Support and deliver services with an anti-oppression, harm reduction lens.
  • Collect pertinent data to support reporting requirements.
  • Provide patients with resources to other community services including addiction, mental health and primary care as needed.
  • Assess, document, and implement actions required to meet patients and their families social, emotional and support needs.
  • Work with various families and groups as is required to provide supportive services.
  • Work with patients of all ages, abilities, and life circumstances in varying and unpredictable situations.
  • Demonstrate leadership ability and has experience and willingness to lead change.
  • Provide outreach and harm reduction services. Engages patients' significant support systems. Activities include assessment of patient and families social, emotional and support needs, intake/admission, evaluation, triage, teaching, and monitoring of patient engagement.
  • Track and analyzes data related to patient population and develops a plan of action and monitors progress over time and initiates changes as needed.
  • Communicate with a multidisciplinary team (physicians, advanced practice clinicians, nurses, behavioral health consultants, etc.) as needed to assist with disease management.
  • Utilize patient-centered communications strategies, e.g., motivational interviewing.
  • Involve the patient in developing a plan of care, goals or other specific measures pertinent to their health condition.
  • Actively participate in all leadership team activities.
  • Act as a liaison between clinics, administration, and community resources, as needed.
Must have's you'll need to be successful:
  • Equivalent education and experience and expertise relevant to a four-year degree in social work, health education, health care management or a similar field of study.
  • Valid Washington State Driver's License with an acceptable driving record determined by HealthPoint's insurance carrier.
  • Beginning level of Word, Excel and Outlook required.
If you know about the following it's a plus:
  • Experience working with asylum seekers and/or refugees is preferred.
  • Ability to speak Spanish is preferred.
Proof of vaccination for COVID-19 is required, prior to start. HealthPoint does not accept the Johnson & Johnson COVID-19 vaccine as proof of vaccination. If you have received the Johnson & Johnson vaccine, we ask that you provide documentation demonstrating proof of an alternate COVID vaccine or vaccine series. All new employees are also required to show proof of immunizations and/or immunity to MMR (measles, mumps, rubella), Varicella, annual Influenza and TB QuantiFERON Gold Titer. Additionally, if you work in a HealthPoint clinic, Tdap (within last 10 years) is required. Hepatitis B. is required for clinical employees with potential exposure to blood/blood products. All immunizations are a condition of employment. Upon hire, employees must provide proof of their immunizations and/or immune titer results prior to starting or no later than their fifth (5) business day of employment. Where to gather your records:
  • If you are providing immunizations from an electronic health record, please ensure that you obtain a copy of your full records rather than a screenshot. Each page of your records should include your first and last name, date of birth, and the name of the health system from which the records are pulled.
  • If records do not show any data, please seek guidance from your provider for further assistance.
  • If you are unable to provide proof as noted above, you can choose to have a lab titer drawn to check immunity or to be re-vaccinated. If you receive vaccination(s) or lab titers, you may obtain them through HealthPoint at no cost to you. This is a great opportunity to get your immunization record up to date at no additional expense.
