Background: The Office of Mental Health (OMH) and the Department of Health (DOH) are co-defendants in the Stipulation and Order of Settlement, United States v State of New Your, Civil Action No. 13-0 CV-4165 (NGG) O'Toole et al v. Cuomo et al., Civil Action No. 13-CV-4166 (NGG). This settlement requires the state to assist residents of impacted adult homes, who have been identified as class members to move out of adult homes into supported apartments if they choose to and are eligible. They will have the assistance of a care manager to ensure any needed wrap around services are in place prior to the move. An assessment determines the care planning needs that the care manager implements as part of the preparation for transition out of Adult Home. Under the general supervision of the Vice President of Supported Housing and Adult Homes is responsible for the oversight of the administrative, operational, staffing, and clinical aspects of the program.
