We are searching for a Supervisor Community Health- Someone to oversee the Field Relations Coordinators (FR - Cs) performing member outreach to improve member wellness at the Health Plan. Ensure productivity of the FRC team, appropriate documentation, and member satisfaction. Handle customer concerns and issues that require a high degree of appropriate knowledge, creativity, research and communication with internal and external resources. Assist leadership by developing relationships with community organizations and furthering the social determinants of health strategy. Think you've got what it takes? Qualifications: Bachelor's Degree required, BS degree in health services, public health, or other health care relevant field preferred. 3 years of population health, community public health, ambulatory care, other clinical care, and/or healthcare admin experience required.
