The Case Managers for the Shelter Into Rapid Rehousing (SIRR) program are responsible for providing housing assistance and services to program participants. Service models include Housing First, Trauma-Informed Care, Harm Reduction, Motivational Interviewing, and Critical Time Intervention. This position collaborates with the Community Housing team at the Bannock Youth & Family Center and involves conducting needs assessments, community outreach, case management, housing navigation, supporting housing retention, connecting clients to income resources, requesting rental assistance, landlord mediation, attending community meetings, and accompanying participants to appointments. The role requires the use of a personal vehicle for participant housing support.
Essential Duties and Responsibilities
- Ensure participants' voices are heard and their ideas influence programming.
- Coordinate wrap-around services with the team and community providers.
- Utilize standardized screening and assessment tools, completing needs-based assessments.
- Attend intakes, housing orientations, lease signings, and educational sessions with participants.
- Meet participants where they are, including in the community, and transport them for case management as needed.
- Collect necessary documents such as IDs, income verification, and household information.
- Assist clients in finding and understanding housing, including filling rental applications and lease interpretation.
- Maintain accurate data entry and documentation per grant guidelines.
- Create safety plans to ensure participant safety and well-being.
- Conduct outreach to identify eligible households facing barriers to housing stability.
- Deliver services equitably, respecting diverse needs and ensuring participant involvement.
- Apply knowledge of program requirements, referral processes, and HUD standards.
- Demonstrate resourcefulness, passion for serving homeless populations, and team collaboration.
- Stay informed on relevant issues and best practices in the field.
- Participate in trainings, community meetings, and professional development activities.
- Provide training and mentorship to team members and community stakeholders.
- Maintain confidentiality and adhere to ethical standards.
- Perform other duties as assigned.

Requirements
- Bachelors degree in human services, social work, or related field, or relevant experience.
- Valid Colorado drivers license and insurance.
Preferred Qualifications
- Knowledge of the local housing market and resources.
- Experience with homeless populations.
- Crisis intervention and de-escalation skills.
Skills and Knowledge

Physical Requirements
- Willingness to use a personal vehicle across the Denver Metro area.
- Work in close proximity to other staff at Bannock Youth & Family Center.
Part-Time Schedule: 20 hours/week, Monday-Friday, 8am-4:30pm, with some evenings and weekends.
Location: 455 Bannock Street, Denver, CO 80204.
Salary Range: $21.00 - $23.00/hour.
