As part of our mission, we are seeking a passionate and dedicated Community Health Worker to join our team.As a Community Health Worker (CHW), you will play a vital role in promoting health and wellness within our community via school and local housing authority partnership. You will serve as a liaison between individuals and community resources, providing support, education, and advocacy to improve health outcomes.The intent of this job description is to provide a representative summary of duties and responsibilities that will be required of positions given this title. Employees may be requested to perform job-related tasks other than those specifically presented in this description.Responsibilities & QualificationsESSENTIAL RESPONSIBILITIES:Conductoutreach efforts to identify community health needs and establish relationships with community members.Provide health education and resources to individuals and groups on topics such asnavigating life stressors and coping skill strategies.Assistyouth community members in navigating the healthcare system, including scheduling appointments, understanding insurance coverage, and accessing financial assistance programs, regardless of insurance.Conducthome visits and assessing individuals' needs and provide personalized support on challenges to school attendance and school performance.Collaborate with healthcare providers, social service agencies, and community organizations to coordinate care and address social determinants of health.Advocatefor policy changes and community initiatives that promote health equity and address underlying health disparities.Maintainaccurate records of client interactions and program activities and participating in data collection and reporting efforts.Attend Community Events to promote and represent IHC.Tracking, evaluating, and reporting data for program.On-site (at assigned school or alternate venue) at least3 days per week.ESSENTIAL QUALIFICATIONS:Commitment to the highest quality of client careMust be 21 years of age or olderBachelor's degree in a human service field from an accredited university and 1 year of related experienceORAssociate's degree in a human service field from an accredited school and 1 year of related experienceBilingual language capacity preferredPassion for supporting youth struggling with social, emotional, and behavioral struggles.Demonstrated ability to model, teach, and support, with excellent interpersonal skills.Sensitivity and respect for diversity in both clients and staff.Solid writing and documentation skills, as well as computer literacy.Maintain strictest confidentiality regarding clients, families, and employees; adhere to all HIPPA guidelines and regulations.Visual and hearing acuity to perform job related functions.Previous experience in community outreach, health education, or social services preferred.Knowledge of local resources and social services available to community members.Ability to work independently and as part of a team, with excellent organizational and time management skills.Italian Home for Children is committed to fostering a diverse and inclusive workplace. This number does not reflect additional compensation-related benefits such as PTO, medical and life insurance, EAP, and the Italian Home for Children 403(b) retirement plan.#JoinIHC
