Employment Type:Full timeShift:Day ShiftDescription:Come join the community outreach team at Mercy Medical Center, part of Trinity Health Of New England!!Position PurposeIn this two year grant the Community Health Worker assists homeless individuals in the local community gain access to key social service and healthcare providers/services to better their health and improve their lives.What you will doImprove access to health and human services for homeless population through in-person interactions, telephone contacts, and coordination of referralsFacilitate or conduct enrollment in available federal, state and local programs to increase access to health care and support servicesMaintain frequent contact with community agencies serving vulnerable homeless and at-risk of homelessness populationsProvide individual & community support.
