Case Manager Provides comprehensive case management services by conducting assessments, developing individualized service plans, monitoring client progress, linking clients to community resources, advocating for access to services, maintaining accurate documentation, and representing the agency in community meetings.Job Responsibilities Conducts intakes and assessments and creates timely individualized service plans for each participant which includes specific measurable short- and long-term goals and objectives. Monitors client's progress through regular visits with clients. Maintains a flexible schedule, including evenings and weekends as needed. Provides short- and long-term case management services to individuals and families needing services. Knowledgeable about community resources and links clients to appropriate resources. Advocates on behalf of clients to ensure access to services for which the client is eligible. Maintains up-to-date and accurate departmental files and databases as required by agency/programmatic policies and procedures, professional standards, and external requirements of all relevant regulatory, licensing or accrediting bodies in the course of performing job expectations. Participates in community meetings and represents the agency regarding services provided as well as providing accurate and timely reporting to management staff of the meeting discussions. This includes parish community meetings.PLEASE NOTE: Essential functions include all other duties and responsibilities as assigned.
