Integrated Care Management Program

The Integrated Care Management Program (iCMP) began in 2006 to support our primary care patients with complex health issues and is one of many programs available through our primary care practices. The program now provides care coordination services to more than 4,000 patients annually. The Integrated Care Management Program (iCMP) is designed to reduce complication by supporting patients on their health care journey. Our iCMP nurses, social workers, and community health workers meet with patients and assess their health and social needs. Together, they identify barriers to good health and create a plan to address them. The team works with our patients and their providers to create a personal care plan to support the patient in achieving optimal health and well-being.

Job Summary

The Opportunity

  • Works closely with individuals and communities to promote and maintain good health.
  • Provides education, support, and outreach services to community members to help them achieve their health goals.
  • Works under the direction of a healthcare professional to provide preventive care and health education to members of the community.
  • Conducts community outreach and engage community members to understand their health needs.
  • Provides individual and group health education and coaching to community members.
  • Assists community members in accessing and navigating the healthcare system, including providing referrals to appropriate health services.
  • Provides basic health screenings and assessments to identify health risks.

Qualifications

Education

  • High School Diploma or Equivalent required
  • Associate's Degree Social Work preferred
  • Bachelor's Degree in related field of study preferred

Experience

  • 1+ years of experience in community outreach, health education or related field

Knowledge, Skills & Abilities

  • Strong communication and interpersonal skills, with the ability to interact effectively with various populations.
  • Demonstrated ability to work effectively and provide advocacy for all populations and communities preferred.
  • Ability to work independently and as part of a team.
  • Basic computer skills, including Microsoft Office and database management.
  • Bilingual preferred.

Onsite Flexible Working Model

  • M-F Eastern Business Hours
  • Onsite at community practices in Greenfield/Northampton
  • Includes patient in-home or physician visits
  • Must be flexible for patient needs and team meetings M-F for onsite as needed
