Lakeside Neurologic is a post-acute and transitional rehabilitation community located in beautiful central Florida. We are currently accepting resumes for the position of Program Case Manager. The facility is located on 900 acres just outside of Wauchula, Florida. Lakeside Neurologic treats pediatric and adult survivors of traumatic brain injury. Responsibilities: Coordinates, directs and supervises the client care to assure procedures, treatments and referrals are completed in a timely manner. At time of admission, collaborates with treatment team members for development of functional outcome goals and discharge planning. Serves as a liaison between clients, family members and funders. Completes daily notes, monthly reports, daily IDT meetings. Participates in monthly treatment team meetings with client and/or family input. Qualifications: BS/BA Degree in health rehabilitation or related field with a minimum of two years experience. Demonstrated abilities in interpersonal oral and written communication and leadership are required. Previous rehabilitation management, funding coordination & insurance benefit management experience is preferred.
