For staff who regularly work 30 or more hours per week, benefits include employers pays up to 80% of health insurance, 100% company-paid dental, 401(k) plan with competitive employer match, Flexible Spending Account (FSA), optional life and vision insurance, 14 paid holidays plus paid time off (PTO).The Enhance Care Management Lead Care Coordinator will provide efficient, comprehensive, and effective ECM services to older adults and disabled members of managed health plans. Duties include conducting a thorough assessment of the clients, developing a written care plan with measurable outcomes, arranging and coordinating services, and client follow-up to monitor the effectiveness of the services arranged and assess the need for additional services.PRIMARY RESPONSIBILITIESProvide outreach and engagement to identify eligible members for ECM.Conduct client intake and in-home assessments using program-specific forms to identify client needs and confirm eligibility.Provide member and family/caregiver support as neededField travel to client's homes and other sites, as needed, up to 50% of the time.ECM responsibilities include the following:Conduct a comprehensive in-home assessment and write a biopsychosocial assessment.Navigating internal and external client tracking systems, assuring complete and accurate documentation.Develop a written Care Plan with measurable outcomes to address and implement an agreed-upon action plan for client needs.Referral and coordination of Community Support in support of care plan goals.Maintain contact with the client to monitor the effectiveness of the service arranged for the client and assess the need for additional services.Write accurate, thorough, specific client assessments, letters, emails, and other communications.Complete data reporting promptly and provide thorough documentation for client files.Prepare for weekly Supervision meetings with the ECM Program Manager and other staff.ESSENTIAL JOB FUNCTIONSAbility to perform biopsychosocial assessments of clients using appropriate techniques.Ability to document client information and maintain client boundaries and confidentiality.Ability to visit clients' homes using a personal vehicle or other means of personal transportation.POSITION QUALIFICATIONSBachelor's degree in social work, Gerontology, Health Administration, Human Services, or related field required.Minimum two years of experience working with older adults.Fluency in English (bilingual in Spanish strongly preferred).Previous experience working with an ethnically and economically diverse population.Previous care management experience is strongly preferred.Patient Coordination and Healthcare experience are strongly preferred.Other RequirementsMust have reliable transportation, a current driver's license, a clean driving record, and proof of auto insurance.Travel is required within Los Angeles County.Must pass a criminal background check.WORK ENVIRONMENTThe Employee may be expected to enter premises during field visits that may be environmentally challenging, including, but not limited to, environments with cigarette smoke, bed bugs, dust, and other allergens.This position is in the corporate office of Wise & Healthy Aging, located at 1527 4th Street, Santa Monica, CA 90401.
